The scenario:
You have just gone through a CAP on-site survey. Inspectors found the following Phase II discrepancies:
1. You do not have PT established for ESRs.
2. You have scored below 80% in the last two PT challenges for urine creatinine.
3. There is no documentation that electrical ground checks have been done on several of your centrifuges.
4. When the remote alarm for the Blood Bank refrigerator was tested, no one from the remote location responded.
5. Several MSDSs are found to be missing from your files.
6. You recently replaced your reticulocyte procedure and the retired procedure is not on file.
7. Outdated alkaline phosphatase reagent is found in the refrigerator and in use on your back-up Chem analyzer.
After you remove the egg from your face, devise a plan of action to bring your lab into compliance. Include who you would task for each “fix” and what documentation they need to provide to you.
(You may have to research CLIA/CAP requirements via the Internet or networking with previous training facilities. )
